"Fast Track Colonoscopy"
For healthy patients who only require average risk screening (45 and over for African Americans, 50 and over for all others) and do not wish to have a consult visit prior to the procedure, we offer a "fast track" option.
Providers need to fill out an "attestation sheet" to document that the patient has no medical problems that will preclude endoscopic evaluation in the ambulatory setting.
The completed forms should then be faxed to our office at 704-878-2022 and one of our medical staff will contact the patient once the information on the form has been reviewed and the preprocedure office visit can be bypassed if appropriate.
